HRT for Women in DFW: Creams, Pellets, Capsules, or Troches — Which Is Right for You?

May 28, 2026 · 9 min read

If you've been talking to your provider about hormone replacement therapy and walked out with more questions than answers, you're not alone. One of the most common things we hear at MR Compounding Pharmacy from women across Grand Prairie, Dallas, Arlington, and the broader DFW area is: 'My doctor prescribed bioidentical HRT — but what does that actually mean for how I take it?' The answer depends almost entirely on delivery form — and that's exactly where a compounding pharmacy makes a difference that a commercial product simply can't.

Why delivery form matters as much as the hormone itself

Two women can be prescribed the exact same hormone — say, estradiol — and have completely different experiences depending on how it's delivered. Absorption rates, onset time, duration, and side effect profiles all vary by route of administration. A compounding pharmacy like MR Compounding can prepare the same hormone in multiple forms, giving your prescriber the flexibility to match the medication to your body and lifestyle rather than forcing your body to adapt to a one-size-fits-all commercial product.

Topical creams and gels

Creams and gels are among the most common forms for estradiol, estriol, and progesterone. Applied to thin-skinned areas like the inner wrist, arm, or thigh, they absorb transdermally — bypassing the digestive system and liver metabolism. Best for women who prefer non-oral delivery, those with GI sensitivities, or patients who need precise dose titration over time. Rotating application sites prevents buildup, and the pharmacist can adjust concentration so you apply a small, comfortable amount — something commercial creams can't customize.

Oral capsules

Compounded capsules contain hormones — most commonly progesterone — in oil-based formulations taken orally, typically at bedtime. Oral progesterone undergoes hepatic first-pass metabolism, which converts some of it into metabolites with a calming, sleep-supportive effect, which is why it's often dosed at night. Best for women whose primary symptom is sleep disruption, mood instability, or anxiety alongside hormonal fluctuation. Compounded capsules can be formulated in precise strengths commercial products don't offer.

Troches (sublingual and buccal)

A troche is a small lozenge dissolved under the tongue or against the cheek. Absorption directly through the mucous membrane lets hormones enter the bloodstream quickly, bypassing the liver entirely. Best for women who want faster onset, prefer not to use creams, or have trouble swallowing capsules. Consistent dissolution technique matters, and troches can be flavored by the compounding pharmacy, which significantly improves compliance. Common hormones include estradiol, progesterone, testosterone, and DHEA.

Vaginal preparations

Compounded vaginal creams, suppositories, and inserts deliver hormones directly to vaginal tissue — the primary approach for genitourinary syndrome of menopause (GSM), including dryness, atrophy, painful intercourse, and recurrent UTIs. Best for women experiencing local symptoms who want targeted therapy with minimal systemic absorption. Compounded preparations allow combined formulations (such as estradiol with DHEA or hyaluronic acid) and personalized dosing commercial products can't match.

Pellets

Hormone pellets are small, rice-sized implants inserted subcutaneously during a brief in-office procedure performed by your provider. They dissolve slowly over 3–6 months, releasing a consistent stream of hormones without daily dosing. Best for women who want 'set it and forget it' delivery and consistent levels. Pellets are not adjustable once inserted, and MR Compounding works closely with DFW providers who offer pellet therapy to ensure a consistent quality supply.
